Some help with terms

Within the profiles page there are terms I do not understand. Can someone please define?
1 - Index
2- Ghin Id
Also what is the "enter stats" box for?

Thanks for the reply.
I have never really established a handicap (except for the game of golf itself) and was hoping to establish it this year through Golfstakes. Does the Index then come from your handicap?
