What is SSL and TLS?

Asymmetric cryptography, also known as public-key cryptography, is the cryptographic system that uses a pair of 2 different keys (hence the name asymmetric) during the encryption and decryption processes.
One key in the pair can be shared with everyone and is thus called the public key. The other key is kept secret, so that only the intended recipient can decrypt the message. This key is called the private key.

Methods
By using Taiwan’s National Health Insurance Research Database (NHIRD), we conducted three separate case–control studies of lip cancer, non-lip non-melanoma skin cancer and melanoma. Cases (n = 29,082) with a first-ever skin cancer diagnoses (2008–2015) were matched 1:10 to population controls. We estimated odds ratios (ORs) associating hydrochlorothiazide use with skin cancer risk by using conditional logistic regression.
Results
Hydrochlorothiazide use showed no overall association with any of the three outcomes: ORs for high cumulative use of HCTZ (≥50,000 mg) were 0.86 (95% CI 0.09–7.81) for lip cancer, 1.16 (95% CI 0.98–1.37) for non-lip NMSC and 1.07 (95% CI 0.65–1.76) for melanoma. There was some evidence of a dose–response pattern for non-lip NMSC, with an OR of 1.66 (95% CI 0.82–3.33) for 100,000–149,999 mg of HCTZ. The null findings were robust across subgroup and sensitivity analyses.

Controlling hypertension with medication is known to help reduce the risk of these life-threatening events. Several medications are available to treat hypertension. A dose of indapamide lowers the blood pressure for 24 hours or more, so patients only need to take the medicine once a day. However, if adverse effects happen they can last for a long time. After an overdose, unwanted effects such as excessively low blood pressure, dehydration, weakness, and confusion can continue for up to 3 days. A shorter duration is expected if side effects occur after normal doses.
With regular prescribed use, the most common side effects of indapamide are fluid loss and low blood potassium. If the potassium level drops too much, a potassium supplement might be prescribed. You can also add potassium-rich foods to your diet such as bananas, oranges, avocados, spinach, watermelon, and sweet potatoes. Seniors and patients with heart disease should take special care to drink plenty of fluids to guard against dehydration while taking a diuretic.
If an adult unintentionally takes an extra dose of their indapamide, it is not likely to be a problem. Maintaining good fluid intake and monitoring at home for symptoms of low blood pressure (such as dizziness and nausea) are recommended. Safe single doses can range up to 5 mg in most adults, or up to 2.5 mg in those aged 65 years or older. If a child unintentionally gets into indapamide, treatment will depend on age. Since this drug has not been studied in children, monitoring in a healthcare facility might be needed.

The local hemodynamic effect of pindolol, a nonselective beta-blocker with intrinsic sympathomimetic activity, was investigated in 17 healthy volunteers. Changes in forearm blood flow (FBF) in response to infusion of drugs into the brachial artery were measured by plethysmography. Pindolol increased FBF dose dependently to a maximal value of 62 +/- 8% (mean +/- SEM, p less than 0.001) without inducing changes in heart rate or blood pressure. The pindolol-induced increase in FBF was reduced by concomitant infusion of propranolol (p less than 0.001). Intra-arterial infusion of practolol did not influence FBF. No significant influence of pindolol was found on the vasoconstriction induced by the alpha 1-adrenergic receptor agonist methoxamine, the alpha 2-adrenergic receptor agonist BHT-933, or angiotensin II. Measurement of plasma pindolol concentrations in the venous effluent of the forearm suggested that vasodilatation occurred at drug levels within the therapeutic range. The drug pindolol was proven to reduce alcohol consumption, in particularly binge drinking, according to the study from Queensland University of technology researchers. The good news is, if the drug is proved successful following the completion of human clinical trials, then it already has Food and Drug Administration (FDA) approval. Neuroscientist Professor Selena Bartlet, from the tnstitute of Health and Biomedical Innovation, said it would provide an inexpensive approach to treating alcohol dependence.

The discovery of a new target for the blood-pressure medication propranolol may lead to the development of new and safer therapies for vascular diseases, according to new findings published in eLife.wisepoqder Propanolol powder
The study also helps explain how propranolol is able to shrink benign tumours in infants called hemangiomas and relieve symptoms in some individuals born with a rare condition called hypotrichosis-lymphedema-telangiectasia and renal syndrome (HLTRS), which causes an overgrowth of blood vessels.
Propranolol's ability to block adrenaline receptors has made it a mainstay of treatment for hypertension and other heart-related conditions for decades. More recently, the drug has been repurposed to treat hemangiomas, although it was not clear how it is able to shrink the tumours. But then an adolescent patient with unusually mild symptoms of HLTRS, which is caused by mutations in a gene called SOX18, was found. The patient had been taking high doses of propranolol since a young age to control high blood pressure.
"The milder symptoms seen in this HLTRS patient taking propranolol raised the possibility that the drug has a SOX18-dependent molecular mode of action in addition to its beta-blocking activity," says co-lead author Jeroen Overman, a Ph.D. student at the Institute for Molecular Bioscience, The University of Queensland, Australia.
When a 17-month-old patient in Dubai with HLTRS and a mutation in SOX18 developed heart problems, his physicians and parents chose to try propranolol. This treatment rapidly resolved the child's heart problem, further bolstering the idea that the drug targeted SOX18.
The team conducted experiments in cells grown in the laboratory, and confirmed that propranolol directly interferes with SOX18 activity by preventing it from binding with other SOX18 proteins. Later studies in mice, whereby the researchers treated eight-day-old mice that had SOX18 mutations with propranolol, revealed that the drug eliminated the HLTRS-like overgrowth of blood vessels in the cornea usually seen in mice with these mutations.
Finally, a collaborative team at Boston Children's Hospital, led by co-senior author Joyce Bischoff, treated hemangioma cells collected from patients and grown in the laboratory with propranolol. They found that the drug's SOX18-blocking action stopped the differentiation of the tumour cells. Specifically, they showed that one component of propranolol, called the R(+) enantiomer, is responsible for the SOX18-blocking effect, while its mirror image, S(-) enantiomer, had only a weak effect.
"This discovery suggests that it might be possible to treat hemangiomas or HLTRS using only the R(+) enantiomer of propranolol," says co-senior author Mathias Francois, Ph.D., Associate Professor at the Institute for Molecular Bioscience at The University of Queensland. "This would allow for lower doses of the medication or a shorter duration of therapy, and sparing patients from potential side effects related to beta-blockers."
In addition to opening the door for improved treatment for hemangioma or HLTRS, the discovery may lead to the development of new therapies for other conditions that involve excessive growth of blood-vessel cells. "Our work may enable the repurposing of the R(+) enantiomer of propranolol as a treatment for a broad range of conditions which include vascular disorders," concludes Joyce Bischoff, Ph.D., Principal Investigator in the Vascular Biology Program and Professor in the Department of Surgery at Boston Children's Hospital and Harvard Medical School, Boston, US.

Though Facebook was largely expected to announce its first drop in quarterly profits in four years, it didn’t work out that way. Quite the opposite, in fact, as the company posted a year-on-year first-quarter revenue increase of 26%. In turn, the value of Facebook shares went up as much as 5%.
The boost shows that advertisers, the primary source of the social media company’s profit, are not growing weary of the host of scandals in which Facebook is embroiled.
A slap on the wrist for Zuckerberg?
But it might still get worse still for the company’s founder. It appears the Federal Trade Commission (FTC) in the United States is seeking to hold Zuckerberg personally accountable for the privacy breaches of Facebook.
An investigation by the FTC is considering whether Facebook violated a legal agreement with the U.S. government that promised to keep user data private. If found guilty, the fine imposed is expected to top the $22.5 million fine Google received in 2012, by some distance.
Facebook has been reported to have set aside $3 billion—that’s billion with a b—for a potential fine.
A failure to produce a more privacy-focused network
It had seemed Facebook was taking a step toward a more secure platform. At least, the company was making all the right noises. There was to be a shift away from public posts, and the focus would be on private, secure messaging. So the company claimed.

It’s all well and good to promise higher standards, but it means nothing if they are not backed with tangible actions. |
An unprotected server leaked 24 million sensitive housing documents not once but twice
If you live in the U.S. and bought a house sometime over the last decade, your information may be at risk. According to TechCrunch, more than 24 million mortgage and banking documents were exposed not once but twice.
The exposed information included mortgage loan agreements, payment schedules, borrowers’ phone numbers, and other sensitive financial data.
The culprit? A single unsecured server. If you think that’s bad, it gets worse: In addition to housing millions of sensitive documents, this server didn’t even include a password.
In other words, this information was available to anyone who had five seconds to open their browser and type in the URL.
Leaving valuables inside with the front door unlocked
The documents in question were stored by Ascension, a third-party data and analytics company. In a public blog post, infosec expert Bob Diachenko, who first discovered the public server, stated that there were more than 24 million records openly available.
The records, which go back more than a decade, housed a whopping 51 GB of OCR (optical character recognition) data. While this type of text is readily readable to the naked eye, it can easily be parsed together to divulge private details.
“This information would be a gold mine for cybercriminals who would have everything they need to steal identities, file false tax returns, get loans or credit cards.,” Diachenko wrote.
The lenders had no idea these documents existed
The exposed server housed tens of thousands of financial documents from a range of different banks and institutions, including Wells Fargo, Capital One, HSBC Life Insurance, CitiFinancial, and more. While the information was somewhat scrambled, it was relatively easy to reconstruct—especially if a person was to use the right tools.
But here’s the kicker: Most of the banks have gone on record saying they have zero affiliation with Ascension. In fact, Wells Fargo went on record stating that it had “no vendor relationship with Ascension since 2010.” HSBC said the same thing.
If you don’t know the recipient—or if an email is automatically flagged as suspicious—it’s best to approach with caution.
While this story is still under development, it serves as a cautionary tale to always enable your password and security settings. Because you can’t trust a random company to protect your personal details, it’s up to you to take your privacy into your own hands.

It’s not immediately clear why the app needs all this data. One can only assume that it’s reporting it all to your deity of choice. Well, that or ad agencies.
UPDATE: A representative of YouVersion wrote to say that the app’s permissions are meant to ensure the “best possible experience for our users” and that the developer, a non-profit, “will not sell users’ data or personally identifiable information.”
3. Any flashlight app
Let’s start this by pointing out that it’s 2019, and nearly all phones have a built-in flashlight feature. If you have a third-party app, at best, it’s pointless.
Much has been reported about the dangers of installing flashlight apps in the past, and there’s really no need to put yourself at risk by having one. But, if you already have a third-party flashlight app installed, check the permissions it has. And you might want to sit down first.
4. Facebook
No great surprise that we are recommending against this one.

Could Facebook’s messenger merger signal dark days for WhatsApp users?
Facebook founder Mark Zuckerberg recently announced plans to merge his three dominant messaging services: Instagram, Facebook Messenger, and WhatsApp. The goal, according to Facebook, is to create a more unified service across all three platforms.
“We want to build the best messaging experiences we can; and people want messaging to be fast, simple, reliable and private,” a Facebook spokesperson said.
With these services integrated, users will be able to easily message one another from different apps. But while the merger may make these services more convenient, it doesn’t necessarily mean they’ll be more secure.
Encryption as an afterthought
The decision to merge these services has both lawmakers and privacy advocates scratching their heads. Zuckerberg has already stated plans to make each of the three messaging apps encrypted, but the real question is how.
WhatsApp was one of the first messaging services to offer end-to-end encryption, while Messenger offers customizable encryption features that need to be turned on manually. Instagram, however, offers no such protections (or options).
Will Instagram and Messenger be retooled to offer the same privacy settings as WhatsApp? Or will WhatsApp be downgraded with weaker security software to help it function like the other apps?

The second problem is that the increased access to metadata. As the article points out, WA users register with a telephone and FB users don’t. I suspect FB has already done a lot to link these identifiers together, but cross-app comms will be even more effective. 7/

Women with metastatic hormone receptor-positive breast cancer live longer when they receive a combination of two anti-estrogen drugs, fulvestrant and anastrozole, compared with treatment with anastrozole alone, according to results of a SWOG Cancer Research Network study appearing in the New England Journal of Medicine.wisepoqder Anastrozole
These study results provide the most promising evidence to date of a life-extending first line of treatment for the advanced form of hormone receptor-positive breast cancer, which accounts for about two out of every three cases of breast cancer diagnosed in the United States, according to American Cancer Society statistics.
"These results are very exciting," said Rita Mehta, MD, a member SWOG's breast cancer research committee, a clinical professor at the University of California Irvine, and the medical director of the Breast Center at Chao Family Comprehensive Cancer Center. "Women who received fulvestrant, right up front, lived longer based on this new long-term analysis. This is credible evidence that combination endocrine therapy should be considered an option for first-line treatment of advanced hormone receptor-positive breast cancer."
The new results report the final survival outcomes of women who volunteered to participate in S0226, the SWOG trial led by Mehta. Original results, published in the New England Journal of Medicine in 2012, showed an unprecedented improvement in two key cancer treatment measures—how long it takes for cancer to progress, known as progression-free survival, and how long people with cancer live, or overall survival.
Initial results showed that, when women receive an injection of fulvestrant, a hormone treatment, along with doses of anastrozole, an aromatase inhibitor, they had a relative 20 percent increase in progression-free survival and a relative 19 percent increase in overall survival when compared with treatment with anastrozole alone. Anastrozole works by reducing the body's production of estrogen, the hormone that this type of breast cancer needs to grow. Fulvestrant works by disabling the cancer's ability to use estrogen. Mehta thought that these two anti-estrogen actions might be more effective than anastrozole alone, and launched S0226 to test her theory.
Mehta's new results confirm her early ones. The SWOG team followed S0226 volunteers for a median of seven years after starting treatment on the trial, which enrolled 707 postmenopausal women with breast cancer at 73 hospitals, clinics, and cancer centers across the U.S. and Canada. Here's what they learned: When women got the drug combination as their first line of treatment, their lives were extended by eight months compared with women who took anastrozole alone (median overall survival of 50 versus 42 months). In addition, 42 percent of women who got the combination treatment were alive five years after their treatment, compared with 33 percent for women who got anastrozole alone.
What is particularly interesting, Mehta notes, is that patient survival was strongly positive despite the fact that women received lower than normal doses of fulvestrant in the trial—250 mg per month after the first loading dose compared with the typical 500 mg a month. Another important finding: Women who had received no prior hormone treatment before joining S0226 had the best survival outcomes, as did women who had a breast cancer diagnosis more than 10 years prior to joining the trial—regardless of their tamoxifen use.
Finally, Mehta said, the results show that treatment timing matters. About 45 percent of women who got anastrozole only were later treated with fulvestrant when their cancer got worse or spread. But it was the women who received fulvestrant first who had the best outcomes, as measured by both overall survival and progression-free survival.

In prior studies, anastrozole (Arimidex, AstraZeneca) exhibited the ability to suppress estrogen production, while fulvestrant (Faslodex, AstraZeneca) demonstrated high efficacy in a low-estrogen environment. In addition, the combination of fulvestrant and an aromatase inhibitor — compared with either agent alone — delayed the development of resistance by down-regulating several signaling molecules involved in the development of resistance.
To determine whether the combination of anastrozole and fulvestrant would be superior to anastrozole alone as first-line therapy for metastatic breast cancer, Rita Mehta, MD, and colleagues from the UC Irvine Medical Center conducted a phase 3 randomized trial of 694 postmenopausal women with previously untreated metastatic disease from June 1, 2004, to July 1, 2009.
During the study, patients were randomly assigned to receive either 1 mg of anastrozole orally once daily in combination with injections of fulvestrant — given in sequential dosing on the first day, every 2 weeks, and then once every 28 days after the first month — or 1 mg of anastrozole orally once daily, with possible crossover to fulvestrant alone following disease progression.
Median PFS was 15 months for patients who received the anastrozole and fulvestrant combination vs. 13.5 months for patients who received anastrozole alone, according to study results. The combination was observed to be generally more effective than anastrozole alone in all subgroups, with no significant interactions.
In addition, median OS was longer among patients assigned to the combination therapy (47.7 months vs. 41.3 months), despite the fact that 41% of the patients in the anastrozole group crossed over to fulvestrant after progression.
“The improvement in overall survival that was observed in our study has not been seen in other trials of first-line hormonal therapy for HR-positive metastatic breast cancer,” the researchers wrote. “Specifically, in the trials comparing aromatase-inhibitor therapy with tamoxifen therapy, the benefit from aromatase inhibitors with respect to progression-free survival failed to translate into a benefit with respect to overall survival, a finding that was attributed to the crossover of some patients in the tamoxifen group to an aromatase inhibitor.”
Three deaths that were possibly associated with treatment occurred in the combination group. Toxic effects of grade-4 or higher were observed in four patients who received anastrozole alone (1.2%) and in five patients who received combination therapy (1.4%; P=1.00). The four observed grade-4 toxic effects among patients who received anastrozole alone included thrombosis or embolism, joint pain, thrombocytopenia and dyspnea. Two patients in the combination group experienced grade 4-toxic effects. One experienced thrombosis or embolism, and the other experienced neutropenia or lymphopenia.
“The results of our study suggest that trials of adjuvant therapy should be performed in which the combination of an aromatase inhibitor and high-dose fulvestrant is compared with an aromatase inhibitor alone or high-dose fulvestrant alone in patients with estrogen-receptor–positive tumors for whom chemotherapy is not necessary,” the researchers concluded.

The results come from the ABCSG-16 trial, a randomized controlled trial of almost 3500 women reported here at the San Antonio Breast Cancer Symposium (SABCS) 2017.
Lead investigator, Michael Gnant, MD, director and chairman of the Department of Surgery, Comprehensive Cancer Center, Medical University of Vienna, Austria, said in a release: "There is simply no rationale to keep most patients on extended AI for longer than two years."
"This result can help save a lot of unnecessary side effects for many women around the world," he said, adding: "I believe that these trial results should be implemented into daily practice at once."
Presenting his findings, Dr Gnant began by highlighting that hormone receptor–positive breast cancer has a significant long-term risk for relapse and that more than 50% of relapses occur after the first 5 years of follow-up.
There has consequently been a tendency to extend the duration of adjuvant hormone therapy. The results of several studies exploring how best to extend therapy have shown that after 5 years of tamoxifen, there is a clear benefit from extended adjuvant therapy with AIs, although the picture is less clear after initial AI treatment.
Dr Gnant said, "So, the question remains: If we extended adjuvant aromatase inhibitors, for how long do we have to treat?"
The researchers therefore undertook the ABCSG-16 trial, in which 3484 postmenopausal women with hormone receptor–positive stage I to III breast cancer from 75 centers in Austria were randomly assigned to 2 years or 5 years of anastrazole, 1 mg/day.
All the women had already undergone 4 to 6 years of endocrine therapy with tamoxifen, an AI, or an AI after tamoxifen following initial surgery with or without radiotherapy.

It is in a class of drugs called estrogen agonists/antagonists that have been developed to provide the beneficial effects of estrogens without all of the potential disadvantages. Raloxifene reduces the risk of spine fractures. There are no data showing that raloxifene reduces the risk of hip and other non-spine fractures. For both prevention and treatment, raloxifene is taken daily as a 60 mg tablet, with or without meals.
Raloxifene appears to decrease the risk of estrogen-dependent breast cancer by 65 percent over eight years. It is FDA-approved to decrease the risk of invasive breast cancer in postmenopausal women with osteoporosis and even in women without osteoporosis who are at high risk of breast cancer. Raloxifene does not reduce the risk of coronary heart disease.
Side Effects
Side effects include hot flashes, leg cramps and an increased risk of deep vein thrombosis (blood clots). Other side effects include swelling and temporary flu-like symptoms. Raloxifene is not associated with diseases of the uterus or ovaries and does not affect cognitive (mental) function.
Raloxifene should not be taken by women at increased risk for stroke. This includes women who have had previous strokes, transient ischemic attacks (TIAs), atrial fibrillation (a type of serious irregular heart beat) or uncontrolled hypertension (high blood pressure).
